This notice serves as final acknowledgment that all clients have been discharged from therapy and all cases have been closed. You are free to choose your own provider, and you are not limited to the list given here. You are responsible for taking steps to connect with your new provider.
In accordance with state and federal regulations, medical records will be securely stored and made available to former clients upon written request. In accordance with Logical Interventions policies, and applicable privacy laws, all participants in a couples or family therapy unit must provide written authorization for the records to be released. Hand signed records requests can be initiated via postal mail which should be sent to:

All requests for records must contain the following for all unit participants:
- Legal name
- Physical address
- Email address
- Phone number
- Reason for request
- Consent to be contacted via phone for verification of identity
- Requested record date range
- Record format preference
- Hand signature of all treatment unit participants
Records custodian will follow-up to written requests within the legally allotted time period upon it’s receipt.
